Tournament Check In

WELCOME to the 2017 First Annual CFC Fall Classic Tournament!!

A representative from each team will be required to attend the mandatory team check-in. Check in Will be the Friday November 11, from 5:00pm to 9:00 pm. Check-In will at our Host Hotel: Sheraton Albuquerque Uptown 2600 Louisiana Boulevard NE Albuquerque, New Mexico 87110
Required DOCUMENTATION for Check-In:

If you are a member of USYSA you will need to bring:

  1. For out of state teams not in Region IV: approved travel papers from your State Association
  2. Provide a Copy of your State Roster for us to keep (Add any Guest Players, cross off any players not attending)
  3. Signed guest player permission form, if applicable (copy to be turned in)
  4. Laminated state player passes
  5. Membership Form
  6. Medical release form for each player (including loan players).
  7. Emergency cell phone numbers for your team in case of inclement weather, etc.
  8. Hotel information for teams that are traveling
  9. Signed Waiver, click here to download

If you are a member of US Club you will need to bring:

  1. US Club Soccer Tournament Roster (copy to be turned in)
  2. US Club Soccer laminated player passes
  3. Signed US Club Soccer medical releases
  4. Emergency cell phone numbers for your team in case of inclement weather, etc.
  5. Hotel information for teams that are traveling.
